Updates

Medical Information Update - Please take this form to the child's annual well check. Return completed form and updated immunizations to the Day School office. The state licensing agency requires that we keep all well-check and immunzation records current.

Authorization to Pick Up Addition or Deletion - Bring this completed form to the Day School office when making a change to the pick up list. Remember, no one will be allowed to pick up a child from the classroom without written authorization from the parent.

Address/Phone Number/Email Change - Bring this completed form to the Day School office when changing home address, phone numbers, or email.

Day School 2012-2013 Child Forms Packet

For children who have been accepted for the 2012-13 school year, Forms Packets must be picked up in the Day School office anytime April 30-May 4, 2012. Office hours are 8:30A-6P.

Completed 2012-2013 forms are due in the Day School office Monday, July 2, 2012.

For children that are enrolled after the school year begins, completed forms must be turned in a minimum of three business days before the child may begin attending class.

As a convenience, we will make the forms available for download after packet pickup is complete:

Summer 2012 Child Forms Packet

Summer Forms are required for children who are not enrolled in St. Luke's Day School for the 2010-2011 school year. Completed forms are due in the Day School office by Tuesday, May 15, 2012, regardless of child's start date for the summer.

Packets are available for pickup in the Day School office April 30-May4, 2012.
